Type 2 diabetes is a condition in which the body doesn’t use insulin well, so blood sugar stays higher than normal. Prediabetes means blood sugar is above normal but not yet in the diabetes range, and it is often a chance to make changes early.
Common symptoms may include:
Having one or more of these symptoms does not confirm a diagnosis.
Schedule a visit if you have symptoms, risk factors, or a recent high blood sugar reading, or if you’d like to switch your ongoing diabetes care to a family doctor. Established patients with a sudden spike in blood sugar can call for a same-day visit.
Diagnosis is based on blood tests such as an A1c and fasting blood sugar, done through a partnered lab, along with a review of your health history.
Care may include lifestyle coaching, blood sugar monitoring, A1c tracking, medication management, and connection to diabetes education resources. We coordinate with endocrinology for complex cases.
High blood pressure, or hypertension, means the force of blood against your artery walls stays higher than it should. Over time it raises the risk of heart disease, stroke, and kidney problems.
Common symptoms may include:
Having one or more of these symptoms does not confirm a diagnosis.
Schedule a visit if you’ve had elevated readings or it’s been a while since your blood pressure was checked. Call 911 for chest pain, trouble breathing, or signs of stroke.
Blood pressure is checked during your visit and over time. Your doctor may also review home readings and order lab work to look at related risks.
Care may include lifestyle changes, medication when needed, and regular follow-up to keep readings on track over the long haul.
Colds, flu, and COVID-19 are viral infections of the respiratory tract. Most improve on their own, but some cause more severe illness or lead to complications such as sinus or ear infections.
Common symptoms may include:
Having one or more of these symptoms does not confirm a diagnosis.
Established patients can call for a same-day or next-day visit if symptoms are getting worse, lasting longer than expected, or affecting a chronic condition. Call 911 or go to the ER for trouble breathing or chest pain.
Diagnosis is usually based on your symptoms and an exam. Testing may be recommended depending on your symptoms.
Care may include symptom relief, rest and fluids, and prescription treatment when appropriate. Some low-acuity concerns can be handled by telehealth.
A urinary tract infection (UTI) is an infection in part of the urinary system, most often the bladder. UTIs are common and usually treatable.
Common symptoms may include:
Having one or more of these symptoms does not confirm a diagnosis.
Established patients can call for a same-day visit if you have UTI symptoms. Seek urgent care for fever, back or side pain, or vomiting, which can be signs of a more serious infection.
Diagnosis is based on your symptoms and a urine test.
Treatment usually includes prescription medication when appropriate, plenty of fluids, and follow-up if symptoms don’t improve.
Perimenopause is the transition leading up to menopause, when hormone levels shift and periods become irregular. Menopause is reached after 12 months without a period.
Common symptoms may include:
Having one or more of these symptoms does not confirm a diagnosis.
Schedule a visit if symptoms are affecting your sleep, mood, or daily life, or if you’d like to talk through hormone therapy and other options.
Diagnosis is usually based on your age, symptoms, and menstrual history. Lab work may be recommended in some cases.
Options may include lifestyle approaches, non-hormonal treatments, and an evidence-based conversation about hormone therapy, along with bone health screening.
Anxiety and depression are common health conditions that can affect mood, thinking, sleep, energy, and daily life. They are treatable, and primary care is often a good place to start.
Common symptoms may include:
Having one or more of these symptoms does not confirm a diagnosis.
Schedule a visit if symptoms last more than two weeks or make daily life harder. If you are having thoughts of suicide or self-harm, call or text 988 or call 911 right away.
Your doctor will talk with you about your symptoms and history and may use standard screening questionnaires. Lab work may be ordered to check for other causes, such as thyroid problems.
Care may include counseling referrals, medication when appropriate, and regular follow-up. We coordinate with mental health specialists when needed.
